• Application Letter and Biodata
• TOR (for college/vocational/technical graduates); College ID (for College level); High School Diploma (for high school graduates)
• Permanent SSS#
• Birth Certificate

• Male or Female , 22 years old and above
• BS Pharmacy Graduate
• Young and Dynamic an advantage but not a requirement
• Additional Requirements:
• PTR, Board Certificate, PRC License
